Abstract

Objective

—To evaluate the relationship between HbA1c variability and cognition impairment (CI) in patients with type 2 diabetes mellitus (T2D) and to explore the related influencing factors of CI.

Methods

—A longitudinal cohort study of 426 T2D patients was conducted

follow-up plan quarterly

a total of 2 years of follow-up to complete 8 visits. Intrapersonal mean and CV of all recorded HbA1c measurements were calculated. A1c-CV represented the measure of HbA1c variability. In addition

medical history and clinical data of all subjects were collected and analyzed.

Results

—Compared with the NCI group

patients in CI group had a longer duration of diabetes

were more likely to be complicated with coronary heart disease and stroke

diabetic nephropathy and diabetic retinopathy

and had a higher proportion of insulin use

a higher HbA1c value

a lower proportion of oral hypoglycemic agents

and a lower MMSE score (

0.05). Mixed effect results showed that A1c-Mean

A1c-SD were positively correlated with CI occurrence (β=0.037

Conclusions

—HbA1c variability is an independent risk factor of CI in patients with type 2 diabetes and can be used to predict the occurrence of CI in patients with type 2 diabetes.
